Milano made his first career start against the Cincinnati Bengals on October 8, 2017, and had a total of four tackles in the 20-16 defeat. Strongside linebacker Ramon Humber, who was declared inactive because of a broken hand sustained the week before, was replaced as the starter by him. Up his second straight start on October 22, 2017, he made an outstanding effort, contributing five total tackles, two tackles for a loss, a pass deflection, and his first career interception to the Buffalo Bills’ 30-27 victory over the Tampa Bay Buccaneers. Jameis Winston attempted to complete a throw, but he intercepted it, returned it 15 yards, and was awarded the game ball by head coach Sean McDermott. In the 34-14 victory over the Oakland Raiders in Week 8, Milano recorded four combined tackles and scored his first touchdown of the season. DeAndré Washington of the Raiders fumbled after cornerback Leonard Johnson forced it in the second quarter of that game; Milano recovered it and returned it 40 yards for a score.  The following week, Milano switched back to a backup position while Humber resumed starting at weakside linebacker.
